Birthday expansion for Home From Hospital scheme

A scheme helping people re-adjust to home life after a hospital stay is celebrating its first birthday...with an expansion.
Home From Hospital will be extended across the whole Bradford district following a successful pilot project in Airedale.
The initiative – co-ordinated by the Carers’ Resource – assists people with everything from benefits guidance and restarting home care to ensuring their property is safe and that power and water supplies are in working order.
Support is tailored to individual needs following an assessment and is provided for up to six weeks.
Patients also receive a hamper of basic foodstuffs.
The service will now not only apply to people being discharged from Airedale Hospital, but to those returning home from Bradford Royal Infirmary and St Luke’s, as well as from community hospitals.
Shelley Marshall, who manages the project for the Carers’ Resource, said: “In addition to supporting people to become more confident and comfortable when returning home, it has the added bonus of helping to reduce the number of hospital re-admissions.”
A display raising awareness of the scheme is being staged in Airedale Hospital – at the top of the stairs in the main entrance area – from Wednesday until March 5, between 11am and 2pm. Team members will be present to answer questions.
